Appointments for all location may be made by contacting the main rooms on 9516 2895 during office hours. Out of hours a message and contact details may be left and we will get back to you the following business day. Appointments at The Central Clinic in Warragul may also be made directly on 5622 3377 .
Dr Hugo consults at three convenient locations throughout Melbourne. His main rooms are in Richmond at the Epworth Hospital where he holds various consulting sessions throughout the week. Dr Hugo also has fortnightly sessions at John Fawkner Hospital in Coburg, St. Vincent’s Private Hospital in Fitzroy and The Central Clinic in Warragul. Patients requiring a Colposcopy will have their appointments booked at Richmond or Fitzroy.
When you arrive, you will be asked to complete a Patient Registration Form and Health Questionnaire. If you prefer, you may complete these forms at home and email them before your appointment. At your first consultation Dr Hugo will spend approximately 45 minutes with you taking a thorough history, doing an examination and reviewing all your tests. He will then provide you with all relevant (and not just surgical) treatments and help you make an informed choice.
If you have been referred to us by your doctor, it is important that you bring the referral letter from your GP with you. The referral letter may pass on important information regarding your medical condition and history and means that you will be able to claim a rebate from Medicare for your visit. GP referrals are valid for 12 months, while referrals from other specialists last 3 months.
To assist in your management, please complete the Health Questionnaire and submit this by email before attending your appointment. If you have a fertility based problem, there is also a separate male Health Questionnaire.
If you are attending for fertility reasons, both you and your partner will need referrals and to complete separate Registration Forms and Health Questionnaires.
If possible please bring along reports and/or copies (if available) of any recent tests that have been done in relation to your condition (e.g. scans, X-rays, blood tests etc). Please also remember to bring your current Medicare card and your private health insurance details.
My rooms will generally call in the days leading up to your appointment to confirm you are attending. If for any reason you cannot make or are running late for your scheduled appointment, it would be greatly appreciated if you could call the main rooms to advise us as soon as possible. We can then offer your appointment to another patient and organise your review at another time.
When you make your appointment with us, please check that we have your most current contact details on file. This means that we can contact you in a timely manner to pass on test results and discuss ongoing management plans. Unfortunately given the nature of our business the doctor may from time to time need to spend longer on a consultation or in theatre or need to attend an emergency. If we can reach you then we can minimise the inconvenience to you.
Dr Hugo is very aware of the costs of modern medical care and the need for you to be able to plan ahead. We try as much as possible to keep fees at a manageable level and to communicate costs ahead of time.
